Which policy change do you wish to make?

Change of Information

Terms & Conditions:

The insured (member) is eligible to request changes to policy information as outlined below:

  • Change of personal information e.g., name, surname, address, telephone number and signature.
  • Change of information regarding beneficiary.
  • Request for reprinting of insurance certificate (to replace the lost one).


Required Documents:

  1. Amendment Requisition Form Download
  2. A copy of identification card or passport of the insured (member) with certified true copy.
  3. Certificate of name change issued by government official (in case change of name or surname of the insured (member) or beneficiary
  4. House registration (in case of change of beneficiary’s information).
  5. A copy of Police Report in case of insurance certificate loss.

Remark

Please cover your "blood type" and "religion" information on the copy of your I.D card to comply with the PDPA law. The company has no intention of collecting, using, and disclosing your personal information in this respect.

Turn-around Time: Approximately 5 working days after the company receives the documents completely.

Customers can change policy information by following channels.

  1. Send documents through insurance agent/selling agent.
  2. Send documents to Prudential Life Assurance (Thailand) Public Company Limited through the following channels:

    2.1 Visit Prudential Head Office at Mitrtown Office Tower, 30th Floor from Monday - Friday (except public holidays) during 8.30 -17.00 hrs.

    2.2 Postage mail to Prudential Life Assurance (Thailand) Public Company Limited at 944 Mitrtown Office Tower, 10th, 29th–31st Floor, Rama 4 Road, Wangmai, Pathumwan, Bangkok 10330.

    2.3 Email submission to hotline@prudential.co.th

Cancellation within Free Look Period

Terms & Conditions:

Insurance certificate for Group car loan protection (relevant for insurance plans with cancellation conditions) can be cancelled within 30 days from the date of receiving it (Free Look Period). During this period, the insured member can exercise his or her right to cancel it, and the insurance company will refund the premium. However, if the insured member has already made claims under this policy, he/she is not eligible to cancel it.

Required Documents:

  1. Amendment Requisition Form Download.
  2. A copy of identification card or passport of the insured (member) with certified true copy.
  3. Original insurance Certificate (A copy of Police Report in case of certificate loss.)

Turn-around Time: Approximately 15 working days after the company receives the documents completely.

Customers can request cancellation within the Free Look Period via the following channels.

  1. Send documents through insurance agent/selling agent.
  2. Send documents to Prudential Life Assurance (Thailand) Public Company Limited through the following channels:

    2.1 Visit Prudential Head Office at Mitrtown Office Tower, 30th Floor from Monday - Friday (except public holidays) during 8.30 -17.00 hrs.

Policy Surrender

Terms & Conditions:

If the insured member pays off the loan in full before the due date while the policy is still in force, they have the right to terminate the insurance certificate. In such cases, the company will pay the surrender value (if any) to the insured member, which will be less than the amount of the sum insured stated in the insurance certificate.

Required Documents:

  1.  Surrender Form Download
  2. Bank statement that demonstrates an available credit limit without any outstanding debt. The statement should bear the official seal of the bank.
  3. A copy of passbook of the insured (member) or legal representatives (in case of juvenile) with certified true copy.
  4. Original insurance certificate (A copy of Police Report in case of insurance certificate loss. )
  5. A copy of passbook of the Insured with certified true copy

Remark
