Health and Accident
  • Claim form Download
  • Original medical receipt
  • Medical Certificate
Within 15 days from the company received all required documents completed.
Critical Illness Claims
  • Physician’s Statement, using company’s form Download
  • Pathological Report or Other Laboratory Report. Ex. X-Ray report, CT Scan report   
Within 15 days from the company received all required documents completed.
Disability
  • Physician’s Statement, using company’s form Download
  • X-Ray report and Picture position   
Within 15 days from the company received all required documents completed.
Death

Natural cause of death (Illness)

  • Medical certificate with the Company form Download
  • Physician’s statement
  • Copy of Insured’s household registration and Insured’s ID card (marked “dead” and certified true copy by beneficiary and show original
  • Copy of household registration and ID card of all beneficiaries, with certified true copy and show original

Unnatural cause of death (Accident or other cause)

  • Copy of daily report from Royal Thai Police, certified by a police officer 
  • Copy of Post Mortem Examination Report

For beneficiary – Child, add these following documents

  • Copy of legal representative’s household registration and legal representative’s ID card, with certified true copy (attach the court order appointing a guardian in case the legal representative is not father or mother)
Within 15 days from the company received all required documents completed.

 

Filing a claim directly to the Company

  • By register mail to Prudential Life Assurance(Thailand) Public Company Limited, Claim Department, Address no. 9/9 @sathorn Building, Floor 20th-27th, South Sathorn Road, Yannawa, Sathorn, Bangkok 10120 or
  • At Prudential Thailand’s Service Counter at Headquarters, Floor 24, or at all branch offices or
  • Through servicing agents 

 

Filing a claim through PRU CARE CARD

Note

  • If you have purchase other policies/rider such as daily hospital cash benefit or Accidental Indemnity, we will provide benefits as agreed in the policy automatically to you upon discharge. There is no need to submit documents to the Claims Department except in special cases where we will inform you.
