Critical Illness Insurance Claim

Step 1: Prepare Your Documents

Original medical expense receipt and medical expense summary
(For critical illness riders that include medical expense coverage)

Laboratory test results, such as: 

  • Biopsy report
  • MRI results
  • CT Scan results
  • Other relevant laboratory findings

Government‑issued identification of the insured

Consent form authorising the release of the insured’s medical information (Click)

Step 2: Complete the Form

  • Insured Person’s Section – To be completed and signed by the insured or parent/guardian
  • Attending Physician’s Report – To be completed, signed, and stamped by the treating doctor and hospital

Step 3: Submit Your Claim and Documents

You can submit your claim through any of the following channels:

Claim Processing Time

The Company will complete the claim assessment within 15 days from the date all required documents are received.
In cases where additional investigation is necessary, the assessment will be completed within 90 days.

Identity Verification Requirement (per Anti‑Money Laundering Office regulations)

According to the notification from the Anti‑Money Laundering Office (AMLO), if the claim payment to any beneficiary is THB 100,000 or more, the beneficiary is required to complete identity verification by either:

  • A recent color photograph (front-facing, clearly showing the face, taken within the last 6 months), and
  • A clear color photograph of the National ID card

These documents can be sent to the Company by postal mail.

For beneficiaries who are minors and do not have a National ID card, please provide identification details and verification of the father, mother, or legal guardian.
