Policy Administration for Car Hire Purchase Protection Plan

Provision

Life insurance certificates of Smile Car Extra, Smile Car Extra for CYC plans can be canceled within 30 days from the date the Insured/ receiver receives the certificate (Free Look Period). The Insured has right to cancel a life insurance certificate and get refunded premium from the Company. However, if the Insured has already exercised his/her rights in making claims from this policy, he/she shall not be entitled to cancel the life insurance certificate.

 

• Required documents:

  1. Additional Amendment Requisition Form – Policy Cancellation (within 30 days from the date of receiving a life insurance certificate from the Company) Download
  2. Copy of ID card (certified true copy by the Insured)
  3. Original Life Insurance Certificate (In case of loss, a copy of Police Report on lost documents is required)

 

• Turnaround Time: Approximately 7 working days, from the date the Company receives complete documents/ complete information.

Provision

The Insured has right to surrender the contract/certificate in the event that auto loans are paid off in full beforehand. The amount received will be as shown in the Surrender Value Table (if any), which should be less than the Sum Insured stated in the Life Insurance Certificate.

 

• Required documents

  1. Policy Surrender Requisition Form Download
  2. Bank statement showing loan burden-free status with bank’s seal
  3. Copy of ID card (certified true copy by the Insured)
  4. Original Life Insurance Certificate (In case of loss, a copy of Police Report on lost documents is required)

 

• Turnaround Time: Approximately 20 working days, from the date the Company receives complete documents/ complete information.

Provision

  • Change of personal information – name-surname, address, telephone number
  • Change of information about beneficiary(s)
  • Issuance of new Life Insurance Certificate (replacing the lost one)

 

• Required documents

  1. Additional Amendment Requisition Form Download
  2. Copy of ID card (certified true copy by the Insured)
  3. Other requested supporting documents such as Police Report on lost documents, a copy of household registration (certified true copy by the Insured)

 

• Turnaround Time: Approximately 3 working days, from the date the Company receives complete documents/ complete information.
